Cutting-Edge Products Aiding Endolumenal Procedures

The advancements in minimally invasive procedures are helping physicians to reduce complications, shorten hospital stays, and enable quicker recoveries for patients. However, complex polyp resections are still being done surgically, mainly because of difficult anatomy, high skilled requirements, and lack of stability. For instance, in situations where the polyp is a part of the intestinal wall, the procedure gets challenging for clinicians owing to the flexible structure of the intestines that lead to an unstable setting for endolumenal interventions. To address these challenges, Lumendi, a leading medical device firm, is developing unique products that assist optical visualization, diagnosis, and endoscopic treatment. Lumendi’s products enable less invasive procedures in the gastrointestinal tract, primarily in the colon. Two of the key innovations of Lumendi are the DiLumen EIP and the DiLumen C2 that have been designed to replace various invasive laparoscopic and open surgical procedures being used in leading healthcare organizations or any outpatient setting. The DiLumen EIP and the DiLumen C2 serve as a breakthrough in ensuring better patient outcomes and making endolumenal procedures convenient and easy-to-handle.

The DiLumen EIP is a new and innovative accessory to an endoscope that largely aids the endolumenal therapeutic procedures. The device ensures the correct positioning of an endoscope in the large intestine and assists the procedure by providing best-in-class visualization and diagnosis. Whereas, the DiLumen C2 has been designed to improve stability and manipulation of tissue in the rectum and colon during complex endolumenal procedures. The device consists of a soft flexible sheath that fits over smaller and standard endoscopes. It employs two balloons, one behind the bending section of the endoscope and the second in front of the tip. The balloons create the passage for the endoscope when inflated.

Additionally, the DiLumen C2 also employs two 6mm working channels at the 3 and 9 o’clock position of the endoscope which accommodates the DiLumen Ig grasper and the DiLumen ls scissor. The mechanics of the DiLumen C2 addresses the limitations of the Endoscopic Submucosal Dissection (ESD), like grasping and removing tissue from the intestines.

Apart from the two unique technologies, Lumendi unveiled another state-of-the-art technology, the DiLumen +DR Dynamic Retraction System. The DiLumen +DR is a new element of the fore balloon assembly that facilitates tissue manipulation for resection of polyps and easier dissection, thereby eliminating the need for surgical intervention. The new technology provides best-in-class tissue retraction, continuous tissue tension, and superior visualization of submucosal tissue planes that helps in precise dissection. In addition to this, the DiLumen +DR System can also be attached to a large pedunculated polyp via a detachable noose.

Peter Johann, the CEO of Lumendi says, “The modification has the potential to revolutionize tissue manipulation during both ESD and EMR. Current methods of retraction for ESD involve anchoring a spring to a clip or using floss and a clip to create counter traction along the oral side of the polyp.”

One of the popular cases where DiLumen technology resulted in a profitable solution was that of Patrick Hellen, a patient. In one of Hellen’s routine diagnosis, a large polyp was found in his cecum - a pouch-like organ connected to the junction of the small and large intestines. Things got severe for Hellen when doctors failed to remove the polyp even after two surgeries. With the DiLumen device, doctors got a better sight into the colon remove the large polyp. A follow-up diagnosis showed no re-growth of the polyp, indicating a successful treatment.
