Delivering a Novel Technology for Nocturnal Enuresis

The treatment for nocturnal enuresis is primarily dominated by ‘the 3Ds’—drugs, diapers, and dumb bed pad alarms, according to Jon Coble, Chairman and CEO of GOGO Band. While the drugs stop children from urinating, they have high relapse rates, and diapers only mask bedwetting without solving it. Dumb bed pad alarms are based on outdated 1930’s technology and only alarm once a substantial amount of urine is released and depend on negative reinforcement as a means of providing operant conditioning.

What began as an idea for the co-founders of GOGO Band to prevent their children from bedwetting soon shaped into a bigger mission to assist millions of children and families. It was formed with the help of a team of medical and AI technology experts who designed the GOGO Band system, a novel bedwetting alarm utilizing real-time heart rate variability analysis and AI to stop chronic enuresis.

GOGO Band’s team, through research, realized that by monitoring and analysing the key biometrics of a child, including heart rate variability (HRV) and motion, the right technology can accurately detect and predict their need to urinate in advance. This new found insight enables the system to build a personalized urination model for each child, keeping them dry, and conditioning the neural pathway from the bladder to the brain much deeper and faster than any other existing therapy.

The multi-patented GOGO Band system uses EKG biometric sensor technology in a wearable band worn around the calf and a Bluetooth-enabled moisture sensor that is attached to the front of the child’s pajamas or underwear to time stamp every time a child wets the bed. The system also comes with a bedside tablet PC and an app downloaded to the parent’s smart phone. By leveraging real-time HRV analysis and applied AI and ML, the system alerts the child and parent App to wake up before wetting the bed.

This predictive alarm enables the child to learn the sensation of their bladder filling status before having an accident. It has achieved remarkable results, with children who use the system 80 percent of the time becoming 93 percent dry within 60 days.

The GOGO Band system has three modes of operation— training, predictive, and weaning (lifestyle) modes. In the training mode, the child wears the GOGO Band and the moisture sensor, wherein the system identifies their unique urination biometric patterns throughout two to three weeks. An alert is sent to the GOGO tablet and the parent app, waking the parents immediately to help the child to the toilet with the first sign of moisture. This allows the system to create a unique personalized model for the child based on their enuretic severity and biometric patterns. Following this, the system advances to the predictive mode, actively monitoring the child’s biometric urination signals. When an event is likely to occur, the GOGO Band Tablet receives a ‘wake up’ alert. These alerts help the child wake up, go to the bathroom, and allow them to gradually learn and recognize their urination signal.

In the final stage, the lifestyle mode, the device reduces the number of predictive alerts for the child to wake up independently and ‘hold it.’ Gradually, the child gains more confidence to stay dry independently, and after completing 28 days of dryness independently, the child is entirely free of bedwetting.
This novel solution targets children aged 5 to 18 and can even assist those with deep sleep issues. Looking ahead, GOGO Band aims to cater to the 28 million adults with urinary incontinence.

GOGO Band has become increasingly popular in the medical community and was named one of the top Digital Health companies at a CES in 2020. It was awarded the Medical Device Innovation Award in 2021 by the FDA and the American Academy of Pediatrics. This industry-wide recognition has paved the path for GOGO Band’s increased adoption and “is just the beginning of the journey of transforming millions of lives in the future,” as stated by Coble.
