Ultrasound imaging is ubiquitous to the medical landscape. However, it is yet to make a significant impact in the ophthalmology space due to the challenges of placing an ultrasound transducer on the eye. Although optical coherence tomography and other light-based imaging techniques are considered the standards for imaging the eye, limitations on imaging the entire anterior segment of the eye leaves a major unmet need in ophthalmic diagnostics. The search for a robust solution that can offer high-resolution images of areas such as the entire anterior segment and corneal layers, which proves to be immensely useful in refractive surgeries, ended when ArcScan came into the picture. ArcScan’s intelligent anterior segment imaging system uses high-frequency ultrasound technology and its scanning robot to produce high-resolution images of areas of the eye which are otherwise inaccessible with other existing technologies.

ArcScan combines the best lessons from OCT and other light-based imaging techniques and applies it to ultrasound technology to improve imaging quality and reproducibility, which was previously not available in ophthalmology. “Intelligent imaging and precise measurements along with pre and post-operative diagnostics, especially around the layers of the cornea, are crucial in the refractive surgery space,” says Andrew K. ArcScan’s unique offering, the Insight 100 instrument, presents a comprehensive and precise visualization of the anterior segment of the eye, including areas behind the iris currently inaccessible with existing technologies through its integrated continuous scan.

The solution provides repeatable and reproducible data that enables precision analysis of the entire anterior segment. The Insight 100 instrument lends its benefits to the refractive space through its applications in corneal layer imaging, including epithelium mapping. In refractive procedures where corneal imaging of certain areas is required, the Insight 100 adds value by providing insights deeper into the anatomy of the cornea before and after surgery through its ability to image the area down to the micron level details. This helps surgeons determine the eligibility of candidates for corneal refractive surgeries, or even diagnosing glaucoma and carrying out pre-operative surgical planning for the same. When surgeons need to dig deeper to map out the ciliary body, sulcus, and crystalline lens for the implantation of phakic lenses, ArcScan’s Insight 100’s instrument provides accurate imaging of the area where the lens needs to be implanted.
Compared to optical technology, ArcScan’s Insight System is not limited by pupillary boundaries when it comes to measuring key areas of interest for proper phakic IOL implantation.

Levien narrates a case where one of ArcScan’s patients had trouble understanding a cataract formation and its reason. With ArcScan’s Insight system, they were able to image behind the iris and the crystalline lens and identify a cyst with a unique cataract formation around it quite accurately. In yet another instance, ArcScan’s technology identified a cataract lens implanted upside down, due to which the patient was experiencing irritation and discomfort.

ArcScan is heavily invested in its technology and strongly focuses on engaging with doctors and key opinion leaders to remain reactive to feedback on its technology. As a part of their future endeavors, ArcScan is focused on the development of three ophthalmology applications. Today the team’s primary focus is around phakic IOL implantation, which is an alternative to LASIK procedures. ArcScan is also developing an application for measuring the complete crystalline lens biometry to support next gen cataract lens technologies. Additionally, the company is working alongside leading glaucoma doctors on imaging behind as well as in front of the iris and utilizing the measurements to track the early signs and risks of developing glaucoma. “We have very broad patent protection, and there will be interesting applications outside of ophthalmology eventually,” concludes Levien.

First-of-its-Kind Computer-Controlled Robotic Instrument Receives NMPA Certification
